"Itsuomi's World: Part II" is the fourty-third chapter of A Sign of Affection Manga series. It has not been compiled within volume 11 as of yet in Japan. It is written and illustrated by Suu Morishita.

Itsuomi decided to leave a message on Kousei's blog, causing him to decide to start a conversation via videoconference, and exchange cultural clashes between the Philippines and Germany, although what surprises Kousei is the message he left him, commenting that he is a fan of his. This makes Kousei excited, proposing to Itsuomi to continue exchanging messages.
Kousei works in an NGO that is responsible for the education, health, and well-being of people, especially homeless children.
Itsuomi wants to follow in his footsteps, but it is complicated due to the difference in education and means of an adult compared to a child. However, this does not stop Itsuomi from following in his lead.
At the age of 10, he showed up at the NGO headquarters, which is in Japan, to become a volunteer, physically meeting Kousei for the first time. Kousei also meets Itsuomi's father, who has made a donation, although half of it is from Itsuomi, who decides to keep it a secret by not accepting money from the children.
Since he is still very young, and until he is older he will not be able to do many volunteer activities, he dedicates himself, along with other colleagues from the NGO, to asking for donations, being watched by his father and Kousei.
Itsuomi feels happy to be doing good deeds, and although Kousei suggests going to the Philippines to volunteer, Itsuomi's father decides that it will be when he is older, to which Kousei agrees. Seeing that Itsuomi was excited about it, he suggests that he make a pen pal.
When he returns to Germany, Itsuomi begins to correspond with an 8-year-old Filipino boy named Soan, who thanks him for his donation because he can now go to school.
In the letters they discuss their experiences and dreams.
At age 12, Itsuomi's little sister Amy was born. This fact makes Itsuomi more aware of the fragility of children, deciding to also protect his sister.
Some time later, Kousei and Itsuomi are catching up, telling Kousei that he hasn't received a response from Soan. He seems evasive, but decides to tell him that Soan has died. Without understanding Itsuomi, Kousei tells him that it is common for these things to happen in underdeveloped countries.
Itsuomi asks him why he chose the Philippines. Kousei tells him that it was the first country he traveled to.
Itsuomi asks him what he should do, to which Kousei tells him to study, since knowledge is a shield.
Itsuomi decides to keep the letters he exchanged with Soan, but upon rereading them, he begins to cry. He decides to get a tattoo in memory of the night he decided to fulfill Soan's dream, being a crescent moon and the Wind Rose of the compasses.
With this determination, he decides to go see Kousei, who welcomes him with open arms.
